Facility Request Form

In requesting the facilities at Sharon High School you must complete all the appropriate information on the attached facility request form. This information is vital in meeting your needs and making sure your event is carried out successfully. Failure to check any appropriate box will create a delay in approving your request. In addition, all requests are reviewed on a first come first serve basis. In reviewing your information, please make sure you have completed the following information:

  • Name and address along with a working telephone number in case you need to be reached.

  • Organizational Status (profit/non-profit) and whether a fee will be charged or not.

  • Number of people expected to attend.

  • Which part of the facility you are requesting.

  • If you are having any items sold as a concession - we need to know.

  • Do you need any specific equipment (we may not be able to provide everything)?

  • Date and time of event. (This will not include set up or clean up time by our custodial staff.)

  • Finally make sure you have signed both sides of the request form.

    Any information not provided or requested may result in additional charges and denied permission for such requests on the day of your event.

Once your request is complete, please submit it to Sharon High School and to the attention of Ms. Tia Austin.

I understand the conditions of use and also that final costs will be adjusted on conclusion of the event. I further agree to submit at least two (2) weeks in advance of the event the Certificate of Insurance following approval by the Board in accordance with the content of this proposal.
